As part of the SPOR Network, we envision to develop, evaluate and scale up existing and new integrated primary health care models using a variety of approaches with a specific focus on patients with complex needs. Embedded in the models will be an examination and management of patient and health care provider readiness for change. We will also include new technologies for patient self-management of chronic conditions such as web-based self management support and monitoring programs, alternative care models like group therapy and improved access to information and treatment within specific cohorts and the full scale use of Electronic Medical Records to ensure appropriate distribution of patient records. The short term objectives are to expand the membership of the Network collaborators and partners within and across sectors of care such as, education, social services, mental health, pharmacists, patients, and Canadian and foreign experts to meet the ten SPOR Network requirements and to engage in discussions with other provinces to explore the possibility of an Atlantic SPOR Network. Over the medium term the objectives are to assess the impact of existing integrated primary health care models in the provinces, to develop patient oriented interventions based on the provincial priorities and upstream innovations such as healthy aging, integrated primary health care and self-management of chronic conditions and to develop system oriented interventions based on provincial priorities such as improved access to community based integrated primary health care, urgent care and the coordination of care. The long term objectives are to create a practice based learning and research network integrating horizontal and vertical care with a rapid assessment component using administrative and new data focusing on scaling-up to improve care and reduce costs and to create primary care research capacity.
